Evidence integrity rules

Every captured source is stored with its SHA-256 hash. If the source changes and is re-fetched, a new version is created — the old one is never silently overwritten.
🛡 Tombstoned, not erased
Deleted packs are tombstoned: struck through, unsearchable, and unable to be cited. Even if the same URL is re-fetched later, the tombstone prevents silent resurrection.
📊 Citation-tracked
Each pack records how many runs have cited it. This makes it easy to see which sources are actively used and which are stale candidates for cleanup.
